Playground Awards Ceremony CONSENT FORM
The Triangle is UP for an award
The Triangle are planning a trip to the London Play Adventure Playground of the Year Awards at Rich Mix 35-47 Bethnal Green Rd, London E1 6LA, 10am - 12. It’s always a fun event with popcorn, games and lots of laughs! We are planning to take 15 young people via underground to the event.
The trip will take place on Thursday 31st October Meeting Time 9A.M at the Triangle. Please ensure that your young person brings with them:
1. Oyster Card (if needed): Ensure there is enough for 2 tube journeys.
2. Packed Lunch & Drink: Please bring enough food and water for the morning ( Popcorn will be provided but no drinks provided)
3. Rain Jacket: Please bring a waterproof jacket in case of rain.
4. Medication: e.g asthma inhalers
We should return to the Triangle at the latest by 1 PM. If you would like your child to attend please fill out the link. If you have any queries please do not hesitate to ask one of our Playworkers or contact us on the details below. London Play will be using photographs and films of the event on their website, in press releases to the general media and also for the general purpose of promoting play in London, therefore photo permission is required for your child to attend.
Once a place is given, please give as much notice if your child cannot make it as we do not want to waste a space for another child.
