Football at LBTS CONSENT FORM

The Triangle is running tuesday football sessions at Lilian Baylis

The Triangle will be providing Tuesday football sessions during term time at Lilian Baylis Technology School (LBTS) 323 Kennington Ln, London SE11 5QY. The sessions will run until April 2025. Full dates below

  • Meet at Triangle on Tuesdays at 4pm

  • Walk with 2 staff to LBTS

  • Football session 4.15pm - 5.15pm

  • Back to Triangle for 5.30pm

Please ensure that your young person brings with them:

1. Suitable clothes and trainers/astro truf boots to play football on outdoor astroturf court

2. Water

3. Medication: e.g asthma inhalers
